Hunterdon Health Care System Physical Therapist in Flemington, New Jersey
Position Summary
Provides medically prescribed clinical physical therapy in patients’ homes.
Primary Position Responsibilities
Facilitate and Coordinate Physical Therapy Care.
Evaluates Progress Toward Attainment of Patient Outcomes.
Documentation of Care.
Employs Strategies to Promote a Safe Environment and Patient Health.
Education of the Patient and Care Giver.
Other duties as assigned.
Qualifications
Minimum Education :
Required:
Bachelor’s Degree in Physical Therapy
Preferred:
Master’s or Doctoral Degree in Physical Therapy
Minimum Years of Experience (Amount, Type and Variation) :
Required:
Two years of clinical experience in a hospital, out-patient, or rehab setting.
Preferred:
One year of Home Health experience.
License, Registry or Certification :
Required:
Current license to practice Physical Therapy in the State of New Jersey.
Valid driver’s license.
Current CPR certification. If not certified, must obtain certification within six (6) months of hire
Preferred:
None
Knowledge, Skills and/or Abilities :
Required:
Excellent communication, time management, judgment and decision making skills.
Computer functionality knowledge.
Knowledge and skill in current PT practice.
Preferred:
Knowledge of home health care provision and principles.
