The primary purpose of your job position is to assist with transporting PACE/LIFE participants to and from the

PACE/LIFE Center and to other appointments. To promote safety and comfort of participants while waiting for

transport or appointments. To communicate with drivers and the center, providing relevant information regarding

status of appointments, participants and any problems or issues.

Position Purpose

The Transportation Attendant supports the safe, comfortable, and timely transport of PACE/LIFE participants to and from the PACE/LIFE Center and off-site appointments. This role promotes participant safety and comfort during transport and waiting periods, communicates effectively with drivers, dispatch, and center staff, and provides appointment and participant status updates while addressing any issues that arise.

Responsibilities

  • Demonstrate the Mission, Vision, Values, and Guiding Behaviors of Trinity Health and Trinity Health PACE in all interactions.

  • Foster a calm, respectful, and collaborative environment for participants, families, colleagues, and partners.

  • Assist drivers with safe participant transport in compliance with PACE/LIFE and state licensing regulations.

  • Accompany participants on vehicles and to appointments, assisting with ambulation, toileting, and supervision as needed.

  • Communicate pickups, drop-offs, and updates to the Transportation Supervisor/dispatch and center staff.

  • Support scheduling by providing accurate information to the receptionist regarding attendance and transport needs.

  • Assist with daily vehicle cleaning and sanitizing.

  • Transport supplies, specimens, pharmaceuticals, meals, and mail as assigned.

  • Participate in interdisciplinary team communication; promptly report participant concerns.

  • Attend required training, in-services, and follow all policies, procedures, and compliance standards.

  • Support disaster recovery and business continuity plans as needed.

Minimum Qualifications

  • High school diploma or equivalent required.

  • One (1) year of experience with a frail or elderly population preferred (or willingness to complete PACE training).

  • CPR certification or willingness to be trained.

  • Valid driver’s license in good standing preferred.
