Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.
Team 12 Covers: Albertson, Carle Place, East Hills, East Meadow, East Williston, Elmont, Floral Park, Flower Hill, Franklin Square, Garden City, Garden City South, Hempstead, Herricks, Lake Success, Malverne, Mineola, New Cassel, New Hyde Park, North Hills, Port Washington, Roosevelt, Roslyn, Roslyn Heights, Sands Point, Searingtown, South Hempstead, Stuart Manor, Unionndale, West Hempstead, Old Westbury, Westbury, Queens, Rosedale, Springfield Gardens, St Albans, Woodhaven, Whitestone, Woodside.
5x 8 hour shifts (8:30AM to 4:30PM) - Rotating Weekend

Job Details
Our Home Care LPN’s:
Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.
Measure vital signs, Administer IVs, change dressings, clean wounds as needed
Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.
Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.
Provide patient and family education for disease management and health promotion s directed by physician’s plan of treatment.
Salary Range
USD $33.00 - USD $40.00 /Hr.
This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
