Catholic Health Services Licensed Practical Nurse (LPN) Weekends in Farmingdale, New York

Overview

Catholic Health Home Care is looking for passionate, dedicated LPNs to provide comprehensive care to patients at home.

Team 7: Belle Terre, Centereach, Farmingville, Head of the Harbor, Lake Grove, Nesconset, Nissequogue, Old Field, Port Jefferson, Port Jeff Station, Setauket, St. James, Stonybrook, & Terryville.

  • Weekend Day shift: 7:00AM to 5:30PM

  • Provide 16-18 visits per weekend- ensuring coverage of high priority cases

Job Details

Our Home Care LPN’s:

  • Case manage, assess, plan, develop and implement a comprehensive home health care plan for their patients.

  • Measure vital signs, Administer IVs, change dressings, clean wounds as needed

  • Provides skilled nursing care, preventative rehabilitative procedures, and prescribed treatments with a variety of patient populations within various potentially complex home situations.

  • Work closely with a supportive patient care team including physicians, therapy and social services, pastoral care, home health aides and others for your assigned caseload.

  • Provide patient and family education for disease management and health promotion as directed by physician’s plan of treatment.

  • Minimum Qualifications:

  • New York State Licensed Practical Nurse (LPN) license

  • Minimum of one year of recent hands on experience

  • Basic Life Support Certification

Salary Range

USD $32.00 - USD $40.00 /Hr.

This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
