This Travel RN Case Manager position supports a 571-bed Level II Trauma Center in Everett, Washington, providing comprehensive case management services within an acute care environment. The role focuses on care coordination, utilization review, and discharge planning to ensure safe, efficient, and compliant transitions of care for patients across high-acuity units, including ICU, PCU, Emergency Department, Family Birth, and Adult Medical units. You will collaborate closely with physicians, nursing staff, social work, and interdisciplinary teams to optimize patient outcomes and resource utilization.
Responsibilities
Apply and evaluate admission criteria and medical necessity guidelines to support appropriate levels of care.
Perform utilization review and audits to ensure efficient, cost-effective, and compliant use of healthcare resources.
Provide comprehensive care coordination across multidisciplinary teams, including physicians, nurses, social workers, and post-acute providers.
Lead and manage discharge planning to the appropriate next level of care, including home, skilled nursing, rehabilitation, or other post-acute settings.
Conduct thorough patient and family needs assessments to identify clinical, psychosocial, and resource requirements.
Coordinate and order durable medical equipment (DME) needed for safe discharge and continuity of care.
Develop, implement, and revise individualized plans of care in collaboration with the healthcare team.
Communicate clearly and professionally with providers, patients, families, and post-acute partners to support seamless transitions.
Ensure accurate, timely documentation that aligns with payer, regulatory, and hospital standards.
Utilize EPIC and other clinical information systems to track patient progress, document interventions, and support decision-making.
Apply Milliman Guidelines and InterQual Criteria, as appropriate, to support utilization management and level-of-care determinations.
Essential Skills
Active WA RN license or Compact RN license.
Basic Life Support (BLS) certification.
At least 1 year of discharge planning and care coordination experience.
At least 1 year of inpatient acute care case management experience.
Demonstrated experience in acute care case management within a hospital setting.
Proficiency with EPIC for clinical documentation and chart review.
Experience in utilization review and utilization management as an RN.
Strong skills in RN case management and nurse care management, including care coordination and discharge planning.
Ability to interpret and apply Milliman Guidelines and InterQual Criteria for medical necessity and level-of-care decisions.
Excellent communication and collaboration skills with interdisciplinary teams, patients, and families.
Strong organizational skills with the ability to manage multiple cases and priorities in a fast-paced environment.
Additional Skills & Qualifications
Diverse case management experience across medical-surgical, PCU, ICU, Emergency Department, and family birth or similar units is preferred.
Certified Case Manager (CCM) or Accredited Case Manager (ACM) certification is preferred.
Previous experience in a Level II Trauma Center or similar high-acuity environment is beneficial.
Advanced familiarity with Milliman Guidelines or InterQual Criteria is preferred.
Strong critical thinking and problem-solving skills to support complex discharge planning and resource coordination.
Ability to work independently while maintaining strong collaboration with the interdisciplinary team.
Work Environment
This role is based in a large 571-bed Level II Trauma Center in Everett, Washington, serving high-acuity units such as ICU, PCU, Emergency Department, Family Birth, and Adult Medical. The Case Management team uses EPIC for charting and documentation. Shift options typically include either five 8-hour shifts or four 10-hour shifts per week, with start times for 8-hour shifts generally flexible between 7:00 AM and 8:30 AM, subject to unit approval. Every other weekend is required, and the environment is fast-paced, collaborative, and highly interdisciplinary, with frequent interaction across multiple service lines and post-acute partners.

Pay and Benefits
The pay range for this position is $52.00 - $60.00/hr.
