WorkSource Oregon Intake Coordinator in EUGENE, Oregon

Salary Range: \$20.00 - \$21.20 Per Hour Salary Note: Bilingual candidates are eligible for an additional \$1.50 Bilingual Differential. This Bilingual Differential is not included in the salary range above. Position Summary: The Intake Coordinator is the first contact for new clients referred to Ryan White Case Management Services. This position conducts the initial eligibility screening annual eligibility screening with all clients. The Intake Coordinator is responsible for supporting client access to services including assisting clients in obtaining required eligibility documentation, as appropriate. This position is responsible for scheduling the first appointment for all new clients with a Care Coordinator and with a Medical Case Manager to complete their assessment process. This position also refers to the Care Coordinators and Medical Case Managers when clients need additional assistance after the annual screening is completed. Essential Functions and Responsibilities: Eligibility Assessments: Complete eligibility intakes and annual updates by providing direct one-on-one screening and care services to clients. Able to provide services in-person, over the phone, and on a drop-in basis. Assist with coverage of services. Documentation: Maintain accurate and timely documentation of client interactions, placements, services provided, and progress updates in agency databases. Process and collect necessary eligibility documents including Authorization for Release of Information. Coordination of Services: Work collaboratively with care coordinators and medical case managers, providers, community partners to facilitate access to HIV care and support services. Community Engagement: Build partnerships with local public health authorities and referral agencies. Attend relevant community meetings and participate in ongoing training and professional development. Referrals: Refer client to the Care Coordinator and/or Medical Case Manager for additional assistance as needed after completing eligibility screenings. Travel: Ability to travel to see clients in other community settings and provide home visits on occasion. Ability to transport clients on occasion. Standards: Comply with all applicable program standards, guidelines, laws and administrative rules. Ability to work well with others, handle stress appropriately, have reliable attendance Other tasks as assigned, including training/cross-training other staff as needed Required Qualifications: One year of client interaction experience or two years of administrative experience TB test (provided at agency) Valid Oregon driver\'s license, or valid out of state driver\'s license with the ability to get an Oregon driver\'s license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy.
