Access Coordinator - Central Access

People Incorporated Mental Health Services is seeking an Access Coordinator to support our children's services teams. The Access Coordinator ensures seamless progression for parents and guardians and their children entering services by guiding them through the intake process, supporting initial engagement, and rapidly addressing barriers to access. This role is focused on facilitating timely and efficient connections to appropriate services, acting as a bridge between initial contact and long-term care coordination.

Schedule: Full-Time 1.0 FTE

  • Monday-Friday 10am-6:30pm or

  • Monday-Thursday 8am-6:30pm

Location: 3000 Ames Crossing Road, Eagan, MN 55121 | Hybrid

Hiring Range - $24.30 - $25.90/ Hourly, depending on qualifications, plus $1,000 hiring bonus for external candidates.

Main Job Duties:

  • Serve as the primary point of contact for parents and guardians and their children entering services, establishing early rapport and trust.

  • Guide individuals through the intake process, including completion of pre-screeners and documentation of basic health information.

  • Identify and address immediate barriers to service access (e.g., transportation, system navigation, insurance).

  • Ensure rapid access to comprehensive evaluation and connection to appropriate services.

  • Manage and track internal and external referrals from the evaluation process.

  • Facilitate and coordinate connections to services, including “hand-off” to long-term care coordination teams.

  • Maintain communication with individuals and service providers to ensure successful initiation of services.

  • Continue following client and providing appropriate services until a successful transition to a care coordination program.

  • Develop effective working relationships with internal and external referral resources.

  • Document all interactions, interventions, and relevant information in accordance with organizational and professional standards.

Qualifications

Required Qualifications:

  • High school diploma and:

  • Two years of experience in the delivery of services to people with mental Illness, Traumatic brain injury or developmental disabilities OR:

  • One year of supervised experience in the delivery of services to people with Mental Illness, DD, ID or TBI, and receives clinical supervision from a mental health professional once a week until two years of experience is met

  • OR:

  • High school diploma and:

  • One years of experience in the delivery of services to people with motional disturbance, or substance use disorder, and receives clinical supervision from a mental health professional once a week until 4,000 hours of experience is met

  • One year of supervised experience in the delivery of services to people with Mental Illness, DD, ID or TBI, and receives clinical

  • OR:

  • Has completed 30 semester hours or 45 quarter hours in behavioral science or a related field and

  • Has proof of a practicum or internship that requires direct interaction with adults or children served and is focused on behavioral sciences or related fields;

  • Has 2,000 hours of supervised experience in the delivery of services to people with Mental Illness, DD, ID or TBI

  • OR

  • Bachelor's degree in Behavioral Science or related field

  • Has proof of practicum or internship that require direct interactions with adults or children served and is focused behavioral science or related field

OR

  • Bachelor's degree in Behavioral Science or related field; AND Has proof of 4,000 hours (two years) of experience in the delivery of services to people with mental illness;

  • OR

  • Master’s Degree in behavioral science or related field
