We are seeking a compassionate and dedicated Family Advocate to join our team in Pittsburgh, PA. The ideal candidate will have experience working with families to provide support, resources, and guidance to help them navigate challenges and improve their overall well-being.
Responsibilities:
- Provide direct support and advocacy for families in need.
- Assist families in accessing community resources and services.
- Develop individualized plans to address family needs and goals.
- Facilitate communication between families and service providers.
- Conduct outreach and education to promote family stability and empowerment.
- Maintain accurate documentation and case notes.
- Collaborate with multidisciplinary teams to coordinate care and services.
Qualifications:
- Proven experience as a Family Advocate or in a similar role.
- Strong knowledge of community resources and social services.
- Excellent communication and interpersonal skills.
- Ability to work empathetically with diverse populations.
- Strong organizational and documentation skills.
- Ability to work independently and as part of a team.
