POSITION SUMMARY:
The ID/A Outreach & Systems Navigation Coordinator position is responsible for education and outreach regarding intellectual disability/autism (ID/A) services available through the Office of Developmental Programs (ODP) and assisting applicants with successful navigation and completion of the registration process for ID/A service and supports.
The Outreach & Systems Navigation Coordinator supports ODP Everyday Lives principles, initiatives and promotes understanding of the services and supports available to individuals with intellectual disability, autism, developmental disabilities and medically complex children through outreach events and coordination with individuals/families, school districts, advocacy groups and community partners.
The Outreach & Systems Navigation Coordinator notifies applicants of necessary documentation to establish eligibility, assists individuals/families in requesting/obtaining records and performs tracking, notification, and comprehensive follow-up for intake/registration applications. The position also performs on-going tracking of individuals' eligibility records to facilitate on-going eligibility for ID/A services and supports.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Performs regular community outreach events to educate individuals/families and community members regarding the services and supports available through the Office of Developmental Programs (ODP) and the philosophy, principles and best practices related to ID/A services and supports.
Acts as a department liaison for individuals/families, advocacy groups and community partners regarding stakeholder priorities, systems-barriers and process improvement.
Fields applicant and stakeholder questions concerning ID/A services and supports and translates complex processes and procedures into family-friendly everyday language; fields and responds to all ID/A intake line and Human Services HUB inquires/phone calls; manages the registration system for both intakes and transfers; adheres to strict deadlines for timely data entry to ensure ODP defined timelines are met.
Assists individuals in applying for Medical Assistance through the Department of Human Services (DHS) electronic application process (COMPASS).
Monitors COMPASS for individuals who have expressed interest in ID/A services and outreaches to them regarding the ID/A registration process.
Documents individuals/families' presenting needs at the time of initial contact.
Documents all contacts with individuals and/or families and logs receipt of all information pertaining to an individual's intake in HCSIS, AVATAR, the ID/A drive and any other necessary databases.
Performs necessary initial clearances and creates intake record for all new applicants in applicable Departmental databases and drives.
Sends informational materials and notices of required documentation to intake applicants that include timeframes for document submission. Assists individuals/families in making record requests to organizations with relevant eligibility records; ensures all necessary eligibility documentation is received.
Liaisons with outside agencies for the collection of ID/A eligibility documentation.
Reviews and documents the receipt of required documentat on in applicable Departmental databases and drives.Sends applicants initial, follow-up and final notices concerning necessary documentation submission.
Reviews needs of ineligible applicants and refers to available and appropriate governmental and community supports.
Runs regular application status summary reports in AVATAR and shares necessary follow-up with Intake Coordinators and supervisor.
Upon application completion, assigns individuals to Intake Coordinator for eligibility determination
Upon registration, enters individuals' service preference/eligibility data into the Department functional eligibility database.
Assists Intake Coordinators in timely registration of individuals in HCSIS and transfer to Supports Coordination.
Tracks and ensures incorporation of LifeCourse One Page Description content into registrant Individual Support Plans (ISPs).
Tracks need for Target Support Management Level of Care (LOC) re-determinations, requests updated LOC information and completes re-determinations, as per ODP guidelines.
Assists with completion of annual post-registration reviews of functional eligibility documentation to ensure continued compliance with ODP Eligibility guidance. Notifies individuals, families and Supports Coordinators of post-registration eligibility documentation issues and coordinates necessary follow-up and resolution.
Completes intake eligibility record reviews, as assigned by supervisor.
In concert with supervisor, adjusts or modifies Intake practices based on collected data and analysis to ensure efficiency of delivery of Intake services.
Ensures work processes are optimal for efficient management of the Intake process.
Informs supervisor and Deputy Administrator of Developmental Programs of any individual or family request for an Administrative Meeting.
Participates in State and Department trainings and meetings as requested.
Performs some travel throughout Bucks County and the Southeast Region of Pennsylvania.
Works evenings and weekends as necessary to perform job duties.
