Davita Registered Dietitian in Denver, Colorado

Use experience and education to independently evaluate, develop, and complete comprehensive nutrition assessments and reassessments consistent with CMS regulations. Work with patients and IDT (interdisciplinary team) to establish, document, and update individualized care plans. Assess on-going nutritional status to determine appropriate nutrition interventions and recommendations. Consult physicians regarding lab tests as needed. Develop individualized Medical Nutrition Therapy for patients based on lab results and individualized goals. Lead the development of the treatment and nutritional goals for POC and updates as outlined by the clinic's governing body for stable and unstable patients. Develop and implement nutrition education and counseling based on individualized patient needs including nutrient requirements, fluid allowance, knowledge and abilities, medical, economic, and social situation. Utilizes Motivational Interviewing with patients in need of behavior change on an ongoing basis. Monitors nutritional status, laboratory values, dialysis adequacy, fluid status, adherence to diet and response to nutrition related medications and therapy to determine appropriate nutrition intervention and recommendations. Evaluate nutrition outcomes and fluid status, identifying contributing factors and document interventions. Contact and collaborate with extended care facility staff or other appropriate medical personnel or caregivers to facilitate adequate and appropriate nutritional care. Function as the Medical and Bone Disorder Manager. Effectively communicate pertinent nutrition and fluid issues to the IDT and to patients. Act as a key contributor in patient care conferences, continuous quality improvement (CQI) team meetings, and IDT meetings. Present nutrition related in-service for teammates at least annually. Serve as a role model and potential mentor to Entry Dietitians and in an advisory capacity. Effectively and efficiently handle clinically complex situations. Develop and implement processes to improve clinical outcomes and achieve goals. May serve as an active dietitian preceptor or dietetic intern host. Participate in research studies under the guidance of DaVita Clinical Research if facility is selected to do so. Attend training and on-going education for dietitians and team related programs.  

Position requires a minimum of a master's degree in nutrition, public health, or related field of study and six months of experience in the job or as Clinical Dietician or Nutritionist. Must possess a current registered dietitian license by the Commission of Dietetic Registration. License/certification/accreditation can be obtained within 120-day grace period with Employer.

Domestic travel requirement 30% to meet patient needs, attend trainings, and/or meetings. $78,582 to $87,110 per year.
