Key Responsibilities: Conduct ongoing, holistic assessments of patients and families Develop and manage comprehensive care plans in coordination with the care team Facilitate interdisciplinary case conferences and care planning discussions Collaborate with physicians, hospice staff, volunteers, and external agencies to ensure optimal support Advocate for patients and families, addressing physical, emotional, and psychosocial needs What Youll Bring: Active Massachusetts RN license in good standing Associates degree in Nursing required; BSN or equivalent preferred Proven ability to provide care with compassion and cultural sensitivity in diverse home or facility settings Strong interpersonal skills and the ability to thrive in a collaborative team environment Flexibility, self-direction, and sound clinical judgment Valid drivers license and access to an insured, reliable vehicle If youre ready to bring comfort, clarity, and compassion into the lives of patients and families, wed love to hear from you.
