Responsibilities - Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care.- Completes OASIS assessments timely and accurately.- Provides skilled nursing care to patients in accordance with the plan of care and agency policies.- Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.- Initiates appropriate referrals when indicated.- Promotes and maintains patient health through teaching and appropriate rehabilitative measures.- Assists patients in learning appropriate self-care techniques.- Other duties as assigned. Qualifications - Current RN license, specific to the state(s) you are assigned to work.- One year of clinical experience as an RN.
