Bassett Healthcare Licensed Practical Nurse - Patient Care Coordinator in Cooperstown, New York

What you'll do

Under the supervision of the Oncology Nurse Supervisor, the Patient Care Coordinator LPN coordinates the referral process for medical and radiation oncology patients and obtains access to antineoplastic drugs from specialty pharmacies and pharmaceutical companies. The Patient Care Coordinator bridges coverage gaps for patients to ensure timely treatment and maximize financial benefit for patients. The Patient Care Coordinator is responsible for accurate, timely, and thorough review of referrals, patient advocacy for access to care, and appropriate scheduling and tracking of appointments for patients referred by BCI providers to non-Bassett specialists, clinics and treatment centers. The Patient Care Coordinator provides clinical information to case managers at insurance companies on behalf of the BCI patients to obtain medically necessary treatment and services. The Patient Care Coordinator I also serves as an effective liaison between BCI providers, non-Bassett specialists and clinics, patients, and managed care administrative staff.

  • Provides care and coordinates treatment for Cancer Institute patients

  • Ensures patient referrals within the Bassett Network and to outside medical service facilities are completed in a timely manner as observed by direct supervisory observation and chart documentation

  • Obtains prior authorization for treatment/ therapy plans, and medications to ensure adherence to the prescribed plan of care. Documentation of authorization is completed in the patient record/referral

  • Coordinates on time delivery of cancer drugs to patients through, specialty pharmacies, and local pharmacies

  • Investigates financial resources for patient assistance including pharmaceutical sponsorship, non-for-profit funding, and patient assistance programs and enlists the assistance of social work and financial counselor’s as necessary

  • Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights

  • Delivers care in a non-judgmental and non-discriminatory manner

  • Maintains current LPN licensure

  • Provides to supervisor evidence of completion of all required annual training and competency validation

  • Practices within the New York State-defined LPN scope of practice

  • Performs similar or related duties as requested or directed

What we need from you

Education:

  • Graduation from a Practical Nursing Program, required

    Experience:

  • Minimum 1 year experience in Nursing, preferred

  • Preferably Oncology Nursing

  • Minimum 1 year prior experience with obtaining referrals and accessing patient services, required

    License/ Certification:

  • Current NYS Nursing Licensure as LPN, required

  • RN License would be accepted

  • BLS Certification within 6 months of hire, required

    Physical Requirements:

    We are also offering a sign-on bonus for this position! The bonus amounts are based on the position type and are listed below. Additional details and eligibility requirements will be discussed during the interview process.     Bonus Amount:

  • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time

  • LPN sign on bonuses are awarded in the amount of $10,000 for full time

Pay Transparency

Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. The above compensation range represents a good faith belief of the compensation range by Bassett Healthcare Network, and its entities and affiliates, at the time of this posting or advertisement.

Pay Range Minimum:

USD $25.74/Hr.

Pay Range Maximum:

USD $38.69/Hr.
