Position Title: Home Care Program Supervisor Coolidge Office
Reports To: HCS Branch Manager
About Us:
Ability360 provides high-quality, person-centered, non-medical home care services to AHCCCS/ALTCS/DDD-eligible members in their homes.
Position Summary:
Were seeking a detail-oriented and compassionate Home Care Program Supervisor to oversee Consumer intakes and assessments, match and monitor Personal Care Assistants (PCAs), resolve conflicts, and ensure compliance with AHCCCS and Electronic Visit Verification (EVV) requirements.
Key Responsibilities:
- Conduct Consumer assessments and supervisory on-site visits.
- Assign PCAs based on skill and Consumer needs.
- Monitor PCA performance and provide training.
- Maintain accurate documentation and communication with case managers.
- Support conflict resolution between Consumers and PCAs.
Qualifications:
- 3+ years combined education/experience in case management or home care supervision.
- Knowledge of EVV systems and AHCCCS health plans.
- Bachelors Degree preferred.
- Valid Fingerprint Clearance Card, drivers license, clean driving record, and reliable transportation.
- DDD experience preferred.
- Bilingual a plus.

Schedule:
- MondayFriday, 8:00 AM 5:00 PM, with occasional evenings/weekends.
