Scope of Position/Position Summary
The Patient Care Resource Manager, PCRM, is a nurse case manager that facilitates patients through the health care system to include pre-admission through post discharge coordination of care, resources and services utilizing the nursing process. The PCRM monitors and coordinates the patient plan of care to enhance the timeliness of patient care decisions and throughput and to enhance care quality and efficiency and ensure efficient resource utilization. The PCRM identifies and removes barriers to progression of the plan of care for each patient. The PCRM collaborates with physicians, nurses and other health care workers to manage the patient transition to the next level of care. The PCRM is the central information source for third party payers, communicating the plan of care, medical necessity, and gaining approval for post discharge care. The PCRM, through the use of evidence-based criteria, assists the physicians in ensuring the appropriate level of care for each patient.
Minimum Education Required
Bachelor's Level Degree or equivalent combination of education and experience.
Required Qualifications
Bachelor’s degree, Masters preferred. RN licensure required; five years recent clinical experience applicable to the population preferred; knowledge of clinical practice guidelines, computer data bases, and payer protocols desired.
