PRN Home Health Licensed Practical Nurse (LPN)

What You’ll Do
Deliver skilled nursing care under the direction of an RN or physician
Follow and implement individualized Plan of Care for each patient
Monitor patient conditions and report changes to the care team
Educate patients and families on self-care and safety
Administer medications and maintain accurate documentation
Help prevent avoidable hospitalizations through proactive care
Collaborate closely with RNs and interdisciplinary teams
What You’ll Need
Active LPN/LVN license (state required)
Graduate of an accredited nursing program
1+ year of clinical experience
Home Health or Hospice experience (preferred)
Ability and willingness to travel (50%)
Reliable transportation, valid driver’s license, and auto insurance
Sit, stand, bend, lift and move intermittently and be able to lift 50–100 lbs
