At Morton Plant Rehabilitation Hospital, we specialize in short-term rehabilitation, helping patients recover, regain independence, and safely transition back home. Our RN Case Managers play a critical role in making that happen.
Position Summary
The RN Case Manager serves as the central liaison connecting patients, families, physicians, therapy, nursing, and insurance providers to ensure a safe, timely, and successful discharge home. This role is ideal for nurses who enjoy coordination, communication, and making a measurable impact on patient outcomes beyond bedside care.
Key Responsibilities
Act as the primary coordinator of care, collaborating across interdisciplinary teams (therapy, nursing, physicians, and ancillary services).
Serve as a liaison between the facility, patients/families, and insurance providers to ensure appropriate level of care and coverage.
Lead discharge planning from admission through transition home, focusing on safety, independence, and continuity of care.
Coordinate services such as home health, DME, follow-up appointments, and community resources.
Facilitate care conferences and maintain clear communication with patients and families regarding progress and discharge plans.
Monitor length of stay and barriers to discharge, proactively addressing challenges to ensure efficient and appropriate transitions.
Ensure compliance with regulatory and payer requirements while advocating for patient needs.
Support a culture of teamwork, accountability, and patient-centered care.

• Location: Morton Plant Rehabilitation Center – Clearwater, FL
• Status: Full-Time
• Schedule: Monday – Friday, 8:00 AM – 4:30 PM
• On Call: No
Qualifications
Certifications & Licensure:
• Active Florida RN License (Required)
• BLS Certification (Required)
• Case Management Certification (Preferred)
Education:
• Associate Degree in Nursing (Required)
• Bachelor’s Degree in Nursing (Preferred)
Experience:
• 1 year in Acute Care, Utilization Review, or Home Health (Required)
• 3+ years nursing experience (Preferred)
• Experience in care coordination or discharge planning (Preferred)
