Position Summary:
Northwest Florida Community Hospital is seeking a motivated and detail-oriented Quality & Wellness Nurse (CMA or LPN) to join our medical clinic team. This position supports clinical quality initiatives, preventive care programs, and wellness services to improve patient outcomes. The Quality & Wellness Nurse works closely with providers and clinic staff to ensure patients receive recommended screenings, follow-up care, and education to promote overall health and wellness.
Key Responsibilities:
Assist with quality improvement initiatives and performance measures within the clinic
Review patient charts to identify care gaps and preventive health needs
Contact patients to schedule preventive screenings, wellness visits, and follow-up appointments
Support providers with population health and chronic disease management programs
Track and report quality metrics such as immunizations, screenings, and preventive care compliance
Provide patient education on wellness, preventive care, and chronic disease management
Assist with clinical documentation and quality reporting requirements
Coordinate care with providers, nursing staff, and administrative team members
Maintain compliance with clinic policies, HIPAA regulations, and quality standards
Perform basic clinical duties within scope of certification/licensure as needed
Minimum Qualifications:
Certified Medical Assistant (CMA) or Licensed Practical Nurse (LPN)
Current Florida certification or licensure required
Current BLS certification or ability to obtain within required timeframe
Strong knowledge of medical terminology and clinical documentation
Excellent organizational, communication, and patient interaction skills
Ability to work independently while supporting a team environment
Preferred Qualifications:
Previous experience in primary care or outpatient clinic setting
Experience with quality measures, population health, or preventive care programs
Familiarity with electronic medical records (EMR) systems
Work Environment:
This position is located in a medical clinic setting and requires frequent interaction with patients, providers, and clinic staff. Duties may include computer work, patient communication, and occasional clinical support.
