Nurse Department: Council on Aging Employment Status: Non-Exempt, UFCW 1459 Admin, Full-Time (35 Hours/Week) Pay Range: Grade A11: Step 01 - Dtep 04 - \$28.73/Hour - \$37.61/Hour Schedule: Monday - Friday, 8:00 AM - 4:00 PM Certifications/Licenses Required: Bachelor\'s Degree In Nursing, Current license to practice as Registered Professional Nurse in Commonwealth of Massachusetts. SUMMARY The City of Chicopee\'s Council on Aging Department is hiring a Nurse who will provide on one consultation with older adults and/or their families and/or their physician/pharmacist. The Nurse also provides the same services to frail homebound older adults. Responsible for all record keeping and follow-ups. ESSENTIAL DUTIES INCLUDE: Provides initial assessments to Chicopee residents who are sixty (60) years or older. Follow-up, monitors and makes referrals, as necessary. Counsels clients and/or family regarding health care and problems. Tests blood sugar levels, high blood pressure, anemia, weight loss or gain and/or depression, as necessary. Informs older adults on proper medication management, medication information, instruction, instruction on use of medical equipment, give Vitamin B-12 shots as prescribed by a physician, does dressing changes and does assessment for unhealthy use of alcohol and/or prescription drugs. Provide nutrition counseling based upon evidence-based programs. Keeps accurate health records on each client. Performs physical exams. Informs the Social Services Coordinator and/or other staff as appropriate of any changes in the elder\'s well-being or follow up that might be necessary. Advocates or informs physician and/or family and/or other health services of any follow-up care which the client might need. Orders necessary supplies. Facilities a support group for diabetics. Plans health promotional programs. Provides in-home nursing services for those who have no alternative. Performs other duties as required. QUALIFICATIONS Bachelor\'s Degree In Nursing. - Required Current license to practice as Registered Professional Nurse in Commonwealth of Massachusetts. - Required Three (3) to four (4) years of experience in public or community health nursing or any equivalent combination of education and experience. - Required
