Pay Range: $20.19 - $31.80 per hour

Summary:

The Financial Coordinator-Transition of Care timely transitions of care by coordinating discharge prescriptions between providers, pharmacies, payers, and patients. This role ensures prescriptions processed, adjudicated, and delivered or made available to a patient without delay. The coordinator serves as a central point of communication across inpatient care teams, outpatient pharmacies, and medication access resources to remove barriers related to cost, coverage, and delivery logistics. The position plays a critical role in improving patient outcomes by reducing readmissions and enhancing the overall discharge experience from a Rush facility.

These activities include but are not limited to: providing an estimated patient financial responsibility for discharge prescriptions, identifying, and providing insurance-related information (in-network, contractual information) to inpatient care teams, and coordinating prescription filling and dispensing via meds-to-beds. The coordinator provides excellent customer service to patients and their families. Required Job Qualifications:

• High school diploma/GED required.

• Basic knowledge of medical terminology.

• Possesses a basic understanding of insurance plans (benefits) and contracts (in/out network).

• Understands basic concepts as they pertain to registration and insurance.

• Proficiency with the Microsoft Office Suite.

• Excellent interpersonal skills with the ability to work effectively on an interdisciplinary team.

• Working knowledge of creating and maintaining spreadsheet-based reports is a requirement.

Preferred Job Qualifications:

• Two years of relevant experience required. Previous experience in a medical healthcare environment as a Patient Access Coordinator, Biller/Collector, or Financial Counselor strongly preferred.

• Bilingual proficiency preferred.

Responsibilities:

  • Calculates an estimated patient cost and out of pocket, responsibility, and answers questions regarding patients’ financial responsibility.

  • Accurately documents status, barriers, and viable solutions to patients receiving medications at charge.

  • Administers financial assistance applications in compliance with the department Financial Assistance Program.

  • Accurately makes payment arrangements with patients and supplies supportive documentation.

  • Accurately accepts and applies payments according to policy and procedures.

  • Utilizes all resources available to obtain clinical information in a timely manner that is required to obtain authorization.

  • Obtains all required documentation needed for prescription medication billing, including coordination of benefits.

  • Creates and maintains various reportsrelating to discharge medication access and meds-to-beds services.

  • Identifies benefit limitations and assist patient/responsible parties with financial arrangements for the patient portion of the bill

  • Performs financial counseling for self-paying and assists them on programs offered by the medical center.

  • Refers appropriate patient population to public aid consultants.
