Chatham Hills Subacute Care Center is seeking a compassionate and experienced Director of Social Services to lead our social services department and support the psychosocial needs of our residents. This leadership role is ideal for a dedicated professional who is passionate about advocacy, discharge planning, and resident-centered care in a subacute and long-term care setting.

Position Summary

The Director of Social Services is responsible for overseeing all aspects of the social services program, ensuring residents and families receive comprehensive support in accordance with federal, state, and facility regulations.

Director of Social Services Key Responsibilities

• Conduct psychosocial assessments and develop individualized care plans

• Oversee discharge planning and coordinate post-acute services

• Provide counseling and emotional support to residents and families

• Ensure compliance with CMS and NJ state regulations

• Supervise and mentor social services staff (if applicable)

• Participate in interdisciplinary team meetings and care conferences

• Address resident concerns and grievances

• Assist with admissions and transitions of care

• Prepare for and participate in state and federal surveys

• Maintain accurate documentation and department records

  Director of Social Services Qualifications

• Bachelor’s degree in Social Work required (Master’s preferred)

• Current NJ Social Work license (LSW or LCSW) preferred/required per state guidelines

• Minimum of 2–3 years of experience in a SNF or healthcare setting

• Strong knowledge of discharge planning and long-term care regulations

• Excellent communication, leadership, and organizational skills
