Actalent RN Supervisor in Chatham, New York
Job Title: Registered NurseJob Description
The Registered Nurse will oversee and document healthcare services, medical needs, and interventions for members, participating in Individual Support Plans and case conferences. This role involves advocating for the optimal health of program members and coordinating medical services with Primary Care Physicians. The nurse will ensure medical compliance, manage treatment plans, and maintain confidential medical files for members.
Responsibilities
Conduct preadmission reviews and member assessments.
Develop AFC plans of care.
Recruit, train, supervise, and support AFC caregivers, ensuring compliance with training and care standards.
Perform quality assurance tasks, including audits and policy reviews.
Report high-profile case issues.
Collaborate with other agencies and maintain professional relationships to promote member care and services.
Maintain certifications, licensure, and compliance with all safety, confidentiality, and regulatory standards.
Essential Skills
Patient care and triage.
Medication adherence.
Supervisory experience.
Experience with AFC or similar services.
Proficiency in electronic health records and nursing clinical practice.
Additional Skills & Qualifications
Current RN licensure by the Massachusetts Board of Registration in Nursing.
Minimum of one year of supervisory experience preferred.
Experience in AFC (Home Health) and/or related fields.
Experience with EMR systems, such as Icentrix.
Ability to train adult learners.
Basic computer skills and proficiency in remote communication tools like Zoom.
Valid driver’s license and access to a private vehicle with up-to-date insurance and inspection.
Satisfactory CORI, SORI, and National Fingerprint Check.
Work Environment
The work schedule is Monday to Friday, 8:30 AM to 4:30 PM, with flexibility to stay late as needed. The position offers a work-life balance with schedule flexibility and the potential to work from home once tenured.

Pay and Benefits
The pay range for this position is $80000.00 - $90000.00/yr.
