Join Adoration Home Health & Hospice as a Registered Nurse (RN) Case Manager, where you’ll deliver compassionate, one-on-one care to patients in the comfort of their homes. In this role, you will provide skilled nursing services while coordinating interdisciplinary care to ensure high-quality, patient-centered outcomes. Key Responsibilities:
Conduct comprehensive patient assessments and develop individualized plans of care based on patient and family needs
Provide direct skilled nursing care, including medication administration, treatments, and pain and symptom management
Coordinate care with the interdisciplinary team, including physicians, social workers, therapists, and other healthcare professionals
Monitor patient progress and adjust care plans as clinically indicated
Educate patients and caregivers on disease processes, medications, and care techniques
Provide emotional support and guidance to patients and families
Maintain accurate, timely, and compliant clinical documentation
Participate in on-call rotation as required
Qualifications:
Active Registered Nurse (RN) license in the state of Virginia
Minimum of 1 year of nursing experience required (home health or hospice experience preferred)
Valid driver’s license, reliable transportation, and proof of automobile insurance
Strong communication, assessment, and organizational skills
Ability to work independently and manage a caseload effectively
Must meet all federal, state, and local employment requirements
Must be at least 18 years of age
