Amergis Home Health Registered Nurse Case Manager Needed!! in Charlottesville, Virginia

Job Summary

The RN Case Manager is responsible for providing comprehensive case management services to high-risk members. This role focuses on care coordination, utilization management, and patient advocacy to ensure cost-effective, high-quality outcomes across the continuum of care.

Primary Responsibilities

  • Perform comprehensive assessments of high-risk members to identify clinical, functional, and psychosocial needs

  • Collaborate with primary care providers to develop and implement individualized, comprehensive care plans

  • Identify, recommend, and continuously reassess appropriate and cost-effective levels of care

  • Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and claims professionals

  • Arrange non-medical supports (e.g., housing, transportation) to promote compliance with treatment plans

  • Utilize specialty resources as needed to support optimal patient outcomes

  • Maintain accurate and detailed documentation of clinical, functional, and fiscal outcomes

  • Identify opportunities for health promotion and illness prevention

  • Proactively prevent adverse patient outcomes and intervene promptly when issues arise

  • Advocate for patients while ensuring privacy and confidentiality standards are upheld

Qualifications

  • Associate’s Degree in Nursing or related field required

  • Current, unrestricted Registered Nurse (RN) license in the state of practice

  • Case Management certification preferred or required

  • Strong ability to analyze complex medical information and make sound clinical decisions

  • Experience with Homecare Homebase required

  • Excellent communication, organizational, and care coordination skills

Schedule

  • Thursday through Sunday

  • Pay: $53 an hour with weekly paychecks!!
