Amergis Home Health Registered Nurse Case Manager Needed!! in Charlottesville, Virginia
Job Summary
The RN Case Manager is responsible for providing comprehensive case management services to high-risk members. This role focuses on care coordination, utilization management, and patient advocacy to ensure cost-effective, high-quality outcomes across the continuum of care.
Primary Responsibilities
Perform comprehensive assessments of high-risk members to identify clinical, functional, and psychosocial needs
Collaborate with primary care providers to develop and implement individualized, comprehensive care plans
Identify, recommend, and continuously reassess appropriate and cost-effective levels of care
Coordinate care transitions and collaborate with physicians, social workers, discharge planners, and claims professionals
Arrange non-medical supports (e.g., housing, transportation) to promote compliance with treatment plans
Utilize specialty resources as needed to support optimal patient outcomes
Maintain accurate and detailed documentation of clinical, functional, and fiscal outcomes
Identify opportunities for health promotion and illness prevention
Proactively prevent adverse patient outcomes and intervene promptly when issues arise
Advocate for patients while ensuring privacy and confidentiality standards are upheld
Qualifications
Associate’s Degree in Nursing or related field required
Current, unrestricted Registered Nurse (RN) license in the state of practice
Case Management certification preferred or required
Strong ability to analyze complex medical information and make sound clinical decisions
Experience with Homecare Homebase required
Excellent communication, organizational, and care coordination skills
Schedule
Thursday through Sunday
Pay: $53 an hour with weekly paychecks!!
