What You'll Do
Serve as a clinical leader and patient advocate, supporting safe, efficient, and high-quality care in the ambulatory setting
Perform comprehensive patient assessments that consider physical condition, psychosocial needs, cultural preferences, health literacy, and learning needs
Develop and implement individualized, evidence-based care plans in partnership with patients, families, providers, and interdisciplinary teams
Deliver specialty-specific nursing care, including medication administration, IV starts, injections, wound care, specimen collection, telephone triage, and patient education
Execute provider orders and nursing interventions accurately, safely, and in accordance with established clinical standards
Coordinate referrals, care transitions, and support services to promote continuity of care
Evaluate patient progress, adjust care plans as needed, and document clinical assessments, interventions, and outcomes accurately in the medical record
Communicate clearly and professionally with patients, providers, and care team members, using structured handoff and escalation tools as appropriate
Respond effectively to urgent or emergency situations and prioritize care based on patient needs and changing acuity
Foster a compassionate, respectful, and patient-centered environment through clear communication, inclusion, service excellence, and adherence to organizational values and safety practices
What We're Looking For
Education: High School Diploma or GED, required. 4 Year / Bachelors Degree, preferred.
Graduate of an accredited school of nursing required. If your RN nursing license has been inactive, retired or lapsed for five years or more and you have not been licensed in another state during the last five years, successful completion of a Board approved refresher course is required prior to reactivating or reinstating your license. In order to be considered for current licensure status, you must apply for reinstatement within one year of completing the refresher course.
Experience: One year relevant experience, preferred.
Licensure/Certification: Current RN licensure in appropriate state, required. ACLS, PALS, BAT and DOT, preferred.
