Overview: The Garnet Health Medical Center Catskills is seeking Case Manager in Callicoon, NY.
Coordinates the multi-disciplinary activities related to maintaining continuity of care from the hospital to home setting for Swing Bed, Acute, and Emergency Department Patients. Responsible for discharge planning, utilization review, team conferences and unit-based rounds. Acts as a liaison performing outreach and rounds to hospital and providers to educate on and facilitate Swing Bed admission as a bridge to home.

Requirements:
Registered Nurse with New York State Licensure or masters degree in social work (MSW) from an accredited School of Social Work with Licensure to practice in NYS or eligibility for licensure with valid permit is required. Licensure is expected within 12 months of employment.

Shift and Salary: This is a Part-Time position, Monday - Sunday, First Shift, PDM- Variable days including weekends and holidays, $39.05 - $48.81 hourly
