Job Description:

Monitors the recovery of discharged patients through the Patient Call Back Program. Calls discharged patients at designated times to ensure patient satisfaction and compliance with discharge education. Assesses patient knowledge, compliance, and understanding of discharge instructions through outbound calls. Identifies gaps or barriers in the discharge treatment plan and communicates effectively with the multi-disciplinary team. Provides and reinforces patient and family education as planned by the discharge team. Educates patients and families on disease processes and healthy lifestyle changes. Interacts with clinicians on challenging cases and makes referrals as indicated. Maintains appropriate records and statistics, documenting and tracking findings in the callback database. Contributes to the professional development of self and colleagues, sharing knowledge and skills. Conducts nursing practice ethically, guided by the Code for Nurses, and delivers communication in a non-judgmental and nondiscriminatory manner. Evaluates own nursing practice in relation to professional standards and seeks constructive feedback for improvement. Performs other duties as assigned. Knowledge, Skills, and Abilities:

• Excellent communication and interpersonal skills [Required]

Education:

• Associate [Required]

Field of Study:

• Graduate of an accredited school of nursing

Work Experience:

• 3+ as a licensed vocational nurse [Required]

• Basic level of experience with Microsoft Word and ability to navigate a Windows environment. [Required]

• Skill in assessing, planning, implementing and evaluating patient care as acquired through clinical nursing [Required]
