As a Licensed Practical Nurse (LPN) with our agency, you will deliver compassionate, skilled nursing care to clients in the comfort of their own homes. The ideal candidate is experienced in home care, knowledgeable in using WellSky, and capable of working both independently and collaboratively. Bilingual applicants are highly preferred. This role is perfect for a dedicated professional who takes pride in supporting the health and dignity of those in need. Duties: - Provide compassionate, person-centered care to AFC/GAFC members and support caregivers and direct care aides. - Assist with member assessments, vital signs, medication reviews, and care plan implementation under RN supervision. - Monitor and report changes in member health status promptly. - Support safe medication management and ensure compliance with agency and MassHealth regulations. - Complete accurate and timely documentation in the agency\'s EHR while maintaining confidentiality. - Communicate effectively with members, caregivers, and the interdisciplinary care team. - Follow all safety, infection control, and regulatory requirements. - Participate in required trainings, meetings, and ongoing professional development. - Represent the agency professionally in the community and support outreach/marketing activities as assigned. - Candidates with proficiency in Haitian Creole are preferred. - This may transition to full-time based on program growth and increased service needs.
