Position Summary The Licensed Practical Nurse (LPN) provides high quality, patient centered care in a fast paced, multidisciplinary community health center. The LPN supports providers and the care team by performing clinical nursing tasks, assisting with patient flow, and ensuring safe, efficient delivery of care in accordance with the Nurse Practice Act. Tuition assistance and career growth support Up to \$5,000 sign on bonus Eligibility for State and Federal Loan Forgiveness programs Competitive Paid Time Off (PTO) Key Responsibilities Deliver direct nursing care to patients of all ages following established standards of practice Prepare patients for physicals, sick visits, follow ups, and gynecological exams Obtain and document vital signs, measurements, and clinical information in the EHR Administer medications, injections, treatments, and oxygen therapy Perform CLIA - waived testing, venipuncture, EKGs, nebulizer treatments, and wound care Assist providers during exams and procedures, maintaining sterile technique Support triage activities (telephone and walk-in), referrals, appointments, and nurse visits Provide patient education and support prescription refills Participate in Patient Centered Medical Home (PCMH) activities, including hospital discharge follow ups Ensure proper handling, cleaning, and storage of clinical equipment Participate in quality improvement initiatives, team meetings, and staff training Qualifications Graduate of an accredited school of nursing Current LPN licensure in the Commonwealth of Massachusetts Current CPR certification Ability to maintain all required licensure and certifications Bilingual skills preferred Required Knowledge & Skills Knowledge of nursing principles, disease processes, and clinical assessments Understanding of the Nurse Practice Act and Code of Ethics Familiarity with managed care, OSHA guidelines, infection control, and emergency procedures Basic to moderate computer skills, including electronic documentation Work Environment & Physical Requirements Busy community health center in a downtown setting Long hours may occasionally be required Prolonged standing and walking required Ability to lift up to 40 pounds Visual and hearing acuity sufficient for patient care and communication
