In this role, you’ll partner with the Community Living Manager to provide high-quality, person-centered care. You’ll support daily routines, encourage independence, and help residents achieve their personal goals—all while creating a safe, supportive, and engaging home environment.

What You’ll Do
Support & Empower Residents
Assist with daily living activities (ADLs) such as personal care, meal preparation, and transportation
Encourage independence by teaching and modeling life skills
Support residents in achieving their personal goals and participating in community activities
Help manage social interactions and provide guidance during conflicts when needed
Administer medications and maintain proper documentation
Create a Safe & Supportive Environment
Maintain a clean, safe, and welcoming home
Ensure the health, safety, and well-being of all residents
Follow all HIPAA and organizational policies and procedures
Identify and respond appropriately to emergencies or conflicts
Complete routine safety checks, including monthly inspections
Communicate & Collaborate
Maintain accurate and timely documentation (logs, reports, care plans, etc.)
Build respectful, professional relationships with residents, families, and external partners
Communicate effectively with team members and leadership
Assist with care planning and provide input on residents’ progress and goals

Physical Requirements
This role requires the ability to:
Assist with personal care (including showering and toileting)
Lift up to 50 pounds
Push a wheelchair over uneven surfaces and inclines
Walk, stand, bend, and move frequently throughout the day
Assist residents in and out of vehicles
Work in a variety of weather conditions
