In this role, you’ll partner with the Community Living Manager to provide high-quality, person-centered care. You’ll support daily routines, encourage independence, and help residents achieve their personal goals—all while creating a safe, supportive, and engaging home environment.

    What You’ll Do

    Support & Empower Residents

  • Assist with daily living activities (ADLs) such as personal care, meal preparation, and transportation

  • Encourage independence by teaching and modeling life skills

  • Support residents in achieving their personal goals and participating in community activities

  • Help manage social interactions and provide guidance during conflicts when needed

  • Administer medications and maintain proper documentation

    Create a Safe & Supportive Environment

  • Maintain a clean, safe, and welcoming home

  • Ensure the health, safety, and well-being of all residents

  • Follow all HIPAA and organizational policies and procedures

  • Identify and respond appropriately to emergencies or conflicts

  • Complete routine safety checks, including monthly inspections

    Communicate & Collaborate

  • Maintain accurate and timely documentation (logs, reports, care plans, etc.)

  • Build respectful, professional relationships with residents, families, and external partners

  • Communicate effectively with team members and leadership

  • Assist with care planning and provide input on residents’ progress and goals

    Physical Requirements

This role requires the ability to:

  • Assist with personal care (including showering and toileting)

  • Lift up to 50 pounds

  • Push a wheelchair over uneven surfaces and inclines

  • Walk, stand, bend, and move frequently throughout the day

  • Assist residents in and out of vehicles

  • Work in a variety of weather conditions
