Case Management Coordinator
Location: Field-Based (Must reside in Brandon FL)
Pay Rate: $ 36.07/hr + Mileage Reimbursement
Schedule: Monday – Friday | 8:00 AM – 5:00 PM (Standard Business Hours)
Role Overview
We are seeking a self-motivated, energetic, and tech-savvy Case Management Coordinator to join our Miami-Dade team. This role offers the perfect balance of independence and clinical impact, providing autonomy through a Monday–Friday schedule with no weekend requirements.
You will be responsible for assessing, planning, and coordinating care for a diverse caseload of members, ensuring they receive the support needed to achieve overall wellness.
Key Responsibilities
Field-Based Care (75% Travel): Conduct face-to-face visits with members in their homes throughout Brandon FL
Caseload Management: Oversee a mix of supportive and medically complex members, developing proactive plans to enhance long-term health outcomes.
Care Coordination: Act as a liaison between members, PCPs, and skilled providers; manage prior authorizations and community resource connections.
Telephonic Support: Conduct follow-ups and coordination via phone during office hours.
Required Qualifications
Local Residency: Candidates must live in Miami-Dade County,
Bilingual Fluency: Must be fully fluent in English and Spanish (verbal and written).
Travel: Reliable transportation is required for 75% local travel (mileage is reimbursed to and from member homes).
Technical Skills: Must be tech-savvy and comfortable navigating electronic health records and remote communication tools.

Pay Details: $36.07 per hour
