Intermountain Health Post Acute Document Management Specialist in Bismarck, North Dakota
Job Description:
Performs all tasks related to health information document management, including document pickup, document preparation, scanning and importing of documents, indexing and quality control of documents into the electronic health record. Analyzes health information to determine clinical content and purpose of documents for indexing into the patient medical record according to company policy and Intermountain Health’s designated record set. Ensure health information documentation is available timely for patient care and healthcare operations.

Essential Functions
Responsible for picking up medical record documents from inpatient and outpatient areas for their assigned care sites, accurately prepping, scanning, importing or uploading, and indexing medical records. Must possess the ability and skills to review paper and electronic medical records to determine which documents are appropriate for the medical record.
Responsible for the integrity and quality of documents scanned/indexed into the document management system across their assigned region, i.e. ensuring medical record documentation is processed into the correct patient medical record, correct encounter, and correct document type by reviewing each page for correct patient identifiers
Reviews document content and assigns a document type for accurate display of documents in the electronic record to meet the needs of physicians and other healthcare providers.
Performs quality control to ensure the electronic document/image posts to the correct patient’s visit and location, and all pages are accounted for with corresponding images prior to committing the batch to the EHR system. Adheres to defined quality competencies per policy.
Audits scanned documents and electronic information to identify issues, revise documents and correct errors. Escalates any unresolved issues to Data Integrity or manager per procedure.
Efficiently works with multiple, complex computer programs and hardware components to provide quality checks for both accuracy of content assignment and image quality.
Monitors designated queues and determines appropriate action when necessary.

Required - High School Diploma or Equivalent required
Preferred - Associate or Bachelor's Degree
Certification
Required - None
Preferred - Registered Health Information Technologist (RHIT) or Registered Health Information Administrator (RHIA)
Experience
Required: Previous office experience and/or six months experience in a healthcare setting
Required: Demonstrated proficiency in the use of computers and information systems
Preferred: Experience in a HIM department setting preferred
Preferred: Experience with an electronic medical record
Preferred: Previous experience scanning documentation in any office setting
Physical Requirements
Ongoing need for caregiver to see and read information, labels, documents, monitors, identify equipment and supplies, and be able to assess customer needs.
Frequent interactions with providers, colleagues, customers, patients/clients and visitors that require employee to verbally communicate as well as hear and understand spoken information, needs, and issues quickly and accurately.
Manual dexterity of hands and fingers to manipulate complex and delicate supplies and equipment with precision and accuracy. This includes frequent computer use for typing, accessing needed information, etc.
Occasional lifting 20 lbs. maximum with frequent lifting/carrying up to 10 lbs.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
