Overview
St. Joseph Hospital has achieved disease-specific certification for its Stroke Program, which is designated by NYS as a Gold Plus Center. Other vital services include the St. Francis Imaging Center at St. Joseph Hospital, a state-of-the-art cardiac CT scanner, MRI, Vascular Laboratory, Wound Healing & Hyperbaric Medicine, Center for Sleep Medicine, Center for Swallowing, Speech & Voice, and comprehensive in-patient medical, critical care, and surgical services.
Job Details
The Clinical Pharmacist gathers information for newly approved formulary drugs, as well as monitors the appropriate use of drugs already on formulary. Coordinates the IV to PO program and other Sentri7-related programs. Coordinates the ADE reporting system for the hospital. Administrative duties delegated by the Director of Pharmacy Services are accurately and promptly completed.
Responsibilities:
Participates in the development, implementation, and maintenance of clinical pharmacy services.
Participates in the development and implementation of PI and MUE activities of the department.
Collects and analyzes data related to PI and MUE activities; maintains documentation of all clinical activities.
Presents drug reviews, MUE criteria, and all other clinical reports to the P&T Committee, and other committees as appropriate.
Participates in the development and implementation of treatment guidelines, protocols, formulary changes, and critical pathways as approved by appropriate committees.
Facilitates the identification, resolution, implementation, and communication of cost-effective medication use issues in collaboration with the medical staff.
Supports the pharmacy's pharmacoeconomic efforts. Reviews the use of targeted medications for compliance with current pharmacoeconomic programs.
Provides clinical consultation and clarification to practitioners.
Suggests appropriate therapeutic alternatives, including cost-effective alternatives to medical staff as needed.
Monitors drug therapy regimens for contraindications, drug-drug interactions, allergies, and appropriateness of drug and dose.
Requirements:
Bachelor’s Degree in Pharmacy required; PharmD required.
Licensed as a Registered Pharmacist in the state of New York.
Hospital experience is required
Pharmacy residency is preferred.
Salary Range
USD $71.74 - USD $81.90 /Hr.
