Job Summary
We are seeking a compassionate and dependable PRN Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes. The LPN will work under the supervision of a Registered Nurse (RN) and in collaboration with the interdisciplinary care team to ensure high-quality, patient-centered care.
Responsibilities
Provide direct nursing care to patients in their homes according to the plan of care
Perform skilled nursing tasks such as:
Medication administration
Wound care and dressing changes
Vital signs monitoring and patient assessments
Foley catheter care and other treatments as ordered
Observe, record, and report patient condition and any changes to the supervising RN
Educate patients and caregivers on disease processes, medications, and home care needs
Maintain accurate, timely, and complete clinical documentation in compliance with agency policy and regulations
Follow infection control, safety, and HIPAA guidelines at all times
Communicate effectively with patients, families, physicians, and agency staff
Participate in case conferences or care coordination as needed
Qualifications
Current and active LPN license in good standing in the state of practice
Minimum 1 year of nursing experience preferred (home health experience a plus)
Current CPR certification
Valid driver's license with reliable transportation and current auto insurance
Ability to work independently and manage time effectively
Strong communication and interpersonal skills
Basic computer skills for electronic documentation
Physical Requirements
Ability to lift, reposition, or assist patients as needed
Ability to stand, walk, bend, and kneel during patient care activities
Ability to travel to patient homes
