Patient Outreach Healthcare, Inc. is seeking a compassionate and reliable Licensed Practical Nurse (LPN) to join our growing home health team on a PRN / per diem basis. The ideal candidate will provide skilled nursing services to patients in their homes across Lehigh County and surrounding areas.
Position Summary
The LPN provides skilled nursing care under the supervision of a Registered Nurse (RN) and in accordance with physician orders and agency policies.
This position offers flexible scheduling and the opportunity to make a meaningful impact in patients' lives.
Responsibilities
Provide skilled nursing visits in patients' homes
Perform treatments including wound care, medication administration, and monitoring
Monitor vital signs and patient condition changes
Report changes to supervising RN and physician
Assist with patient and caregiver education
Complete accurate and timely documentation (EMR)
Ensure compliance with state and federal home health regulations
Qualifications
Active Pennsylvania LPN License (Required)
Minimum 1 year of clinical experience (Home health preferred)
Strong wound care and medication management skills preferred
Current CPR certification
Valid driver's license & reliable transportation
Auto insurance
Ability to travel throughout Lehigh and surrounding counties
Excellent communication and documentation skills
