Bay-Arenac Behavioral Health Intensive Case Management Specialist in Bay City, Michigan
Intensive Case Management Specialist Provides intensive, short-term supports to individuals with acute psychiatric symptoms in order to avoid more restrictive care settings and to maximize opportunities for recovery and desired quality of life. Functions as a part of an interdisciplinary team serving adults with serious mental illness. Participates in the Person-Centered Planning team to assist in the coordination and provision of services to individuals within the Agency and the community. Responsible for strength-based recovery assessment, plan development, linking, referring, coordinating, monitoring medications and symptoms, advocacy and administrative duties in support of empowerment and self-determination. Assists in the development and implementation of crisis/safety/relapse plan.
Qualifications Minimum of a bachelor's degree in social work, psychology, or related human services degree. Current Michigan social work licensure preferred. 3-5 years of experience providing service to persons who experience a serious mental illness and/or co-occurring disorders required. Must have a valid, unrestricted driver's license and availability of reliable transportation for outreach work.
Salary Salary range of $46,031 -- $57,911 dependent on status of credential and experience, plus exceptional fringes including employer sponsored training, pension, and insurance. $1,000 signing bonus!
