Pro Action of Steuben and Yates, Inc. Case Manager in BATH, New York

DescriptionCase Manager ($25-$26/hour)The Case Manager will be responsible for case management services and specialized supports with individuals and/or families who are unhoused or housing insecure, are in need of behavioral/mental health and/or addictions services, and who may have other complex social and medical needs. Provides individualized and specialized supports to customers focusing on advocacy, referral, linkages, and follow up for customers to obtain and access behavioral health and/or addiction services, health care, financial assistance, employment opportunities, and housing assistance. This position will be based out of the Steuben County DSS and will require travel to Corning. You should be a professional, caring, and compassionate individuals who has a comprehensive understanding of poverty and have a passion for working with the community. Must have excellent communication, organizational and time management skills and flexibility. Education requirements: LMSW preferred; consideration will be given to related master's level and/or mental health certified candidates (MS. Ed., LMHC, etc.). Experience with case management services and providing direct services to individuals and families in behavioral/mental health and or addiction support services with complex needs and diverse backgrounds is required. 1. The ideal candidate must have a valid drivers license, meeting agency standards per position and consistent, reliable transportation that will allow regular travel in and around the service area.
