Rehoboth Elderly Care and Companionship LLC Home health Registered Nurse Job description 1. Provides professional nursing care to assigned patients in a home care environment 2. Records patient histories, performs various diagnostic tests, administers medications, and develops patient home care plans in conjunction with other medical professionals. 3. Assess patients' condition during every visit and chart your observations 4. Perform evaluation tasks, including vital signs and medication review 5. Administer medication as prescribed by the patient's Physician 6. Operating and monitoring medical equipment as necessary 7. Check bedridden patients for bed sores and muscle weakness. 8. Dress or redress open wounds and assess progress of healing 9. Educate patients and their families on proper home health care strategies and procedures 10. Coordinate with Occupational Therapists, Physical Therapists, Physicians and anyone else involved in the patient's care plan 11. Provide palliative care as needed to keep the patient comfortable 12. Make recommendations for devices or tools that might improve the patient's quality of life 13. Listen to the patient and respond to concerns or requests 14. Identifies care issues and makes recommendations about changes to the patient's care plan 15. Writes and sends reports about each visit to the patient's physician. 16. Typically reports to a manager or supervisor. 17. Requires NYS license to practice. 18. Certification and/or licensing in the position's specialty is the main requirement.
