Position Description
Penn Medicine at Home is seeking a per diem Hospice Nurse Practitioner to join their team! This virtual position will provide weekend (7a-7p), holiday, and off-shift coverage for Pennsylvania Hospital, Chester County Hospital, and community call. The Nurse Practitioner will be responsible for GIP calls, admissions orders, evaluation of IP hospice criteria, and symptom and medication management.
Minimum Requirements
Nurse Practitioner candidates must be licensed to practice professional nursing in the Commonwealth of Pennsylvania, and possess a MSN or DNP and certification as a Adult Gerontology Nurse Practitioner or Family Nurse Practitioner.
The ideal candidate will have hospice experience and at least 3 years' experience as a Nurse Practitioner.
