Recruits, hires, trains, supervises, and evaluates a team of care coordinators and clinical operations support staff. Develops and implements standards and procedures for referral, enrollment, scheduling, and connecting students and families with community resources for long term care. Meets with team members regularly to provide supervision and support. Maintains frequent collaboration with TCHATT colleagues and schools to minimize obstacles and increase patient and provider satisfaction. Maintains confidentiality of information in all aspects of care, identifies and escalates risks, and ensures alignment with all program objectives.Responsible for advanced program administration and coordination clinical support staff. Responsible for all phases of care coordination activities, including development, implementation, and assessment of the clinical program. Oversees a team of care coordinators and clinical operations staff who will collaborate with pediatric psychiatrists, psychologists, other licensed professional counselors, and administrative staff to assist families and patients to navigate through the TCHATT program and assist in connecting to providers in the community.Assists with development and monitoring of policies, procedures, workflows, dashboards, training manuals, templates, and other assets to ensure operational efficiency and effectiveness. Recommends new policies and procedures for improvement of program activities. Responsible for advanced program administration and coordination of medical records management and patient and clinician scheduling. Coordinates staff training and development. Oversees internal and external communication related to patients clinical services, discharge plan and school collaboration among team members, patients and guardians and school staff.Completes documentation in the clinical record related to interaction with individuals-served, families and other service providers in an accurate and timely manner, and in accordance with professional ethics and with both federal and state regulations.Performs routine audits of program communications, documentation, policies, procedures and patient files to ensure staff compliance with program and department policies and procedures. Documents and reports violations and takes corrective action as applicable through appropriate channels. Recommends technical, administrative, or other solutions to minimize risk of violations.Additionally, performs other duties and special projects, as assigned.
