Licensed Practical Nurse (LPN) -- In-Home Patient Care - $5,000 sign-on bonus!

Compensation Highlight:

$5,000 sign-on incentive; hourly pay of $31-38.80/hr

Position Summary

A well-established healthcare organization is seeking a Licensed Practical Nurse (LPN) to provide hands-on clinical care to adult and senior patients in their homes and affiliated care settings. This role focuses on delivering high-quality nursing services, supporting care plans, and collaborating closely with physicians, RNs, patients, and families. Regular in-person visits to patient residences are a core requirement of this position.

Key Responsibilities

  • Deliver direct nursing care in accordance with individualized care plans, professional nursing standards, and regulatory requirements
  • Administer prescribed medications, injections, immunizations, and treatments 
  • Carry out clinical procedures including phlebotomy, TB testing, and vaccine administration when applicable
  • Monitor patient responses to medications and treatments; document and communicate changes in condition to appropriate clinical staff
  • Educate patients and family members on medications, treatments, and care instructions 
  • Accurately complete clinical documentation, progress notes, and required audits using electronic or written systems
  • Execute physician and nursing orders under RN oversight, ensuring proper documentation and follow-up
  • Collaborate with interdisciplinary team members to review and update patient care plans 
  • Participate in quality improvement initiatives, peer review, and medical record utilization reviews
  • Maintain compliance with organizational policies, professional standards, and all applicable state and federal regulations
  • Travel to patient homes to provide care; additional duties may be assigned as needed

Required Qualifications

  • Active Licensed Practical Nurse (LPN) license issued by the State of Colorado
  • Current First Aid and Basic Life Support (BLS) certification (American Heart Association or Red Cross)
  • High School Diploma or GED
  • Valid driver's license, reliable transportation, and current auto insurance
  • Ability to work independently in home-based care environments

Preferred Qualifications

  • 3+ years of experience as an LPN in a clinical, long-term care, or similar healthcare setting
  • Prior experience working with geriatric populations
  • Familiarity with medications commonly prescribed for older adults
