Position Summary
A well-established healthcare organization is seeking a Licensed Practical Nurse (LPN) to provide hands-on clinical care to adult and senior patients in their homes and affiliated care settings. This role focuses on delivering high-quality nursing services, supporting care plans, and collaborating closely with physicians, RNs, patients, and families. Regular in-person visits to patient residences are a core requirement of this position.
Key Responsibilities
- Deliver direct nursing care in accordance with individualized care plans, professional nursing standards, and regulatory requirements
- Administer prescribed medications, injections, immunizations, and treatmentsÂ
- Carry out clinical procedures including phlebotomy, TB testing, and vaccine administration when applicable
- Monitor patient responses to medications and treatments; document and communicate changes in condition to appropriate clinical staff
- Educate patients and family members on medications, treatments, and care instructionsÂ
- Accurately complete clinical documentation, progress notes, and required audits using electronic or written systems
- Execute physician and nursing orders under RN oversight, ensuring proper documentation and follow-up
- Collaborate with interdisciplinary team members to review and update patient care plansÂ
- Participate in quality improvement initiatives, peer review, and medical record utilization reviews
- Maintain compliance with organizational policies, professional standards, and all applicable state and federal regulations
- Travel to patient homes to provide care; additional duties may be assigned as needed
Required Qualifications
- Active Licensed Practical Nurse (LPN) license issued by the State of Colorado
- Current First Aid and Basic Life Support (BLS) certification (American Heart Association or Red Cross)
- High School Diploma or GED
- Valid driver's license, reliable transportation, and current auto insurance
- Ability to work independently in home-based care environments
Preferred Qualifications
- 3+ years of experience as an LPN in a clinical, long-term care, or similar healthcare setting
- Prior experience working with geriatric populations
- Familiarity with medications commonly prescribed for older adults
