Organization: Easterseals MORC
Location: Southfield, MI (multiple locations + community visits)
Schedule: Monday–Friday, 9:00 AM – 5:00 PM
Hours: 40 hours/week
Contract Length: 13 weeks
Status: Contract with potential for extension or temp-to-perm
Pay:
LBSW: $40/hour
LLBSW: $37/hour
About the Role
Easterseals MORC is seeking a compassionate and motivated Case Manager (LBSW or LLBSW) to support individuals living independently in the community. This role is ideal for social workers who are passionate about advocacy, coordination of care, and helping individuals with mental health and developmental needs achieve stability and independence.
Case Managers work closely with a multidisciplinary team to assess needs, connect clients to resources, and provide ongoing support in real-world community settings, including homes, apartments, and outpatient locations.
What You’ll Do
Support adults with mental health and developmental/behavioral needs in community-based settings
Conduct assessments and contribute to individualized care plans
Coordinate services and community resources
Advocate for clients and support goal-setting and independence
Collaborate with clinical teams and external partners
Document care and progress using the Emerge EMR (training provided)
Work Locations & Travel
This role includes travel between multiple Easterseals MORC locations in Southfield, MI , as well as community visits.
Mileage is reimbursed at the IRS standard rate
Valid driver’s license required
Qualifications
LLBSW or LBSW (Michigan licensure/registration required)
Bachelor’s or Master’s degree in Social Work from an accredited program
One year of experience working with individuals with mental illness or relevant fieldwork/internship experience
Strong communication, organization, and advocacy skills
Ability to work independently in the community while collaborating with a team
Training & Support
Week-long orientation provided
Full training on Emerge EMR
Business-casual dress code
Supportive supervision and team-based environment

This is a 13-week temporary contract with the potential to extend or convert to permanent employment (not guaranteed)
