Services Area/Programs: MH/SA Residential

Description of Duties:Meet agency's established productivity standard. Provide medical oversight to MH/SA individuals with co-existing, underlying medical conditions. Meet with individuals at residential sites to coordinate with Psychiatrist, Medical Physicians, Pharmacies, Hospital discharges whether from a CSU, BHCC, or private hospital regarding medications, and discharge. Aid with completing MARS, educating staff and individuals regarding medications whether it is changes and why. Provide nursing assessment to determine individuals mental/medical status and provide injections when needed. Aid and coordination with those individuals that are living individually regarding their medications, completing nursing assessment, education, or medication administration. Provide medical case management for those that are stable on their medications continuing to assist them, monitor, and educate them on their medications and diagnosis. Continue to monitor Medication Case management at least every 90 days or more. Ensure that the individuals have their medications as prescribed by physician and that they have them filled and are taking them correctly. Ensure services are documented and authorized in a timely manner. Provide med education groups. Consult routinely with Primary Care Physicians (PCPs) regarding labs, medications, course of treatment, prescriptions, follow-up appointments. Document individuals' medical goals, objectives, and intervention in an Electronic Medical Record (ECR). Complete nursing assessments and give injections when ordered. Participate in Medical Oversight (MOC) and unit staff meetings. Provide out of clinic nursing services as needed. Completed other nursing duties as assigned. Transporting individuals served is a requirement.

Qualifications:

Bachelor's degree in nursing from an accredited college or university OR Associate's degree in nursing from an accredited college or university AND current Georgia license as a Registered Professional Nurse AND any related professional work experience. Must have a valid, unrestricted Georgia Driver's License. Must be keyboard proficient.
