Description of Duties

:

The individual in this role will serve as a PSH Case Manager

. The PSH Case Manager will work with the Individuals who have a Mental Health and/or Substance abuse diagnosis. The full-time PSH Case manager will work with individuals who have been homeless or chronically homeless. The PSH Case manager is responsible for ensuring individuals are compliant with treatment. They will work with the individual on linking to other services including Primary Care Physician (PCP), day programs for mental health and substance abuse, psychiatric appointments, Social Security office, DFCS, unemployment office, schools, GED, food banks, and other services that are beneficial for the individual to make progress towards their goals. The PSH Case manager will monitor and ensure all problems with home are reported and that the individual is in a safe and clean environment. The PSH Case manager will be responsible for the completion of all documents including housing survey, section 811 documentation and applications for employment. The PSH Case manager will focus with the individual towards whole health and wellness. The PSH case manager will work on stability and independence in each Individual daily lives, identify barriers to treatment and provide support stabilization and recovery. The PSH Case Managers will effectively manage a caseload of up to 50 individuals and complete the minimum requirements of no less than (2) contacts per month with (1) being face-to-face in individuals homes or choice of a safe community setting. The second contact may be a collateral or face to face contact with the individual. They will keep detailed progress notes of visits to home, community locations and phone contacts. All documentation should be completed within 24-48 hours once service has been provided. They will be responsible for meeting the productivity standards for dollar expectations and hourly expectation of 55% or higher each month. They will consult, coordinate, link to services, and assist the individual in what areas are needed for support. They will provide follow up and referral services as needed by completing necessary documentation to ensure Individual receive all services within and outside the agency. The PSH Case manager will serve as liaison between Individuals, their physicians, and other organizational components to ensure communication and linkage for continuity of care of services for the Individual has been accomplished.

Transporting individuals served is a requirement.

Minimum Qualifications:

Associate Degree preferably Bachelors Degree in Psychology, Social Work, Sociology or related field. Must have a valid, unrestricted Georgia Drivers License. Must be computer literate.

Preferred Qualifications:

One

(1) to two (2) years or more experience working with individuals with Behavioral Mental Health AND/OR Substance Abuse Disorders.
