Work hours: Full Time; 8:30am - 5:00pm
Texas Health Harris Methodist Hospital Alliance is a 151-bed, full-service hospital offering acute care and wellness programs to communities in North Fort Worth, Keller, North Richland Hills, Haslet, Justin, and surrounding areas. The hospital provides numerous services, including emergency medicine, general and outpatient surgery - including an outpatient surgery center, women, and infant's care - including a level III neonatal intensive care unit, radiology - including a women's imaging center, cardiac rehabilitation and therapy center, business health services, and wellness programs. What You'll Need
Bachelor's Degree required or Master's Degree Social Work is required
3 years staff nurse at an acute care hospital (if RN) required or
3 years hospital/medical social work (if Social Worker) required or
2 years hospital discharge planning/care management required and
Case Management supervisory background preferred
RN - Registered Nurse upon hire required
LMSW - Licensed Master Social Worker is required upon hire
CPR - Cardiopulmonary Resuscitation (BLS) is required upon hire
ACM - Accredited Case Manager OR CCM - Certified Case Manager is preferred upon hire or other ANCC preferred upon hire
In addition to the qualifications, a successful Care Transition Supervisor will model the factors listed below:
Working knowledge of medical necessity criteria preferred
Knowledge of Microsoft Outlook and Office (Word, Excel)
Customer service skills
Ability to engage in complex clinical decision-making
Strong oral and written communication skills
Strong commitment to interdisciplinary collaboration
Critical thinking, analysis and conflict resolution skills
Flexible scheduling as necessary
Psychosocial and crisis intervention skills
Ability to prioritize and meet deadlines
