JOB REQUIREMENTS: Community Horizons has the privilege of working with a wonderful member of the Algoma community. They are seeking a compassionate and skilled healthcare professional to support them in their home. This individual has complex medical needs and is looking for a reliable, experienced, and attentive caregiver to join their dedicated team. Who They\'re Looking For: A licensed CNA, MA, LPN, or RN who is passionate about providing high-quality, person-centered care. The ideal candidate is dependable, detail-oriented, and comfortable working in a home environment. Position Details: Schedule: 12 shifts per week, with availability for two weekend days per month Shift Hours: 8:00 AM 8:00 PM Compensation: Negotiable, based on licensure and experience Care Responsibilities Include: Vesicostomy Care (straight catheter from bladder to a port in the stomach area) Cecostomy Care (Bowel regime) Trach cares Ventilator protocols G-J tube care Pulmonary care (vest, nebs, humidification) R.OM Massage Medication administration Hoyer lifts (ceiling suspended) Assist with medical appointment Vitals Record keeping (charting) General housekeeping Laundry Supply and equipment maintenance Experience with augmentative and alternative communication (AAC) users and non-verbal individuals The individual utilizes a power wheelchair; familiarity with its operation is advantageous. The selected candidate should also be capable of driving a wheelchair-accessible van. Full, hands-on training will be provided by the current care staff and family to ensure confidence and comfort in all aspects of care. This position is funded through a Medicaid program known as Self-Direction. As a self-directed nurse, you will be employed directly by the family and compensated through Medicaid funds. Please note that this program does not provide the benefits typically associated with agency or larger employers, such as insurance or paid time off. However, the position offers flexibility, the opportunity to build a close one-on-one relationship with the client, and the chance to engage with a friendly family in their home setting while potentially participating in family gatherings, vacations (to Arizona), and meals.
